Passport Rehabilitation
National’s “Passport Rehabilitation” program was specifically designed to meet the needs of individuals requiring a short-term rehabilitative stay following a hospitalization or an acute medical episode. While on your short-term “trip” with us, please take full advantage of the amenities and services our facilities offer. During your stay, if you find anything requiring our attention, please let us as soon as possible so that we may make your stay the best it can be.

Riverside Health and Rehabilitation Center has the capability of providing care to individuals with a wide range of wound care needs. The wound care management program is designed to promote maximum wound healing by utilizing an interdisciplinary team approach to treatment and patient education. The program utilizes a certified wound care consultant in addition to the in-house rehab, dietary and nursing team to provide recommendations for each patient in conjunction with physician consultation. Initial and concurrent treatment modalities and adjunctive therapies are provided to insure the best possible outcomes and healing rates.

Diagnosis and Related Conditions considered for the program:

  • Stage III or IV pressure ulcers
  • Full Thickness Stasis Ulcers
  • Burns - Flaps and Grafts
  • Open Surgical Wounds
  • Wound Infections
  • Osteomyelitis
  • Neuropathic Ulcers
  • Draining Wounds
  • Traumatic Wounds
  • Fistulas/Tubes
Program Description and Goals:
  • Upon referral, evaluate, classify and implement treatment strategies to facilitate healing
  • Interdisciplinary wound and skin team ensures treatment of the entire patient providing Nutritional Therapy, Wound Therapy, Physical and Occupational Therapy as well as Infection Control and Pain Management to facilitate the best outcomes possible
  • Rehabilitation services based on weight bearing status
  • Physician consultation and follow up as indicated ie: plastic surgery, vascular surgery, infectious disease etc.
  • Patient training and family education in preparation for discharge
  • Discharge planning and coordination

This program is designed to provide comprehensive monitoring and management of patients following acute hospitalizations for cardiovascular conditions. This would include those patients that have been treated for medical conditions and/or have undergone surgical procedures.

To ensure a standardized integrated team process for management of patients with a cardiac diagnosis, the program is designed to provide a graded, progressive and predictable format for treatment.

Diagnosis and Related conditions considered for the program:

  • Status Post MI (stable)
  • Status Post Cardiac Catheterization
  • Congestive Heart Failure
  • Peripheral Vascular Disease
  • Recurrent Angina - Arrhythmia
  • CABG
  • Pacemaker
Description and Goals:
  • Monitor of heart rate and blood pressure pre and post activity
  • Monitor of lung sounds and pulse oximetry with oxygen titration
  • Establish activity orders and cardiac precautions per M.D. orders
  • Develop compensatory strategies and education for energy conservation
  • Provide physical therapy treatment and training for mobility, strength and endurance
  • Provide occupational therapy treatment and training for activities of daily living
  • Home assessments to determine adaptations or assistive equipment
  • Discharge planning and coordination of follow up services
  • Cardiology consultation and follow ups as indicated

The Spencer Unit
OUR MISSION is to provide comprehensive medical management and to promote opportunities to individuals with Alzheimer's disease and related dementias. These services are individualized to meet each resident's physical, social, emotional and spiritual needs.

This program represents a unique departure from traditional geriatric long term care. Utilizing specialty selected and educated staff ensures for the provision of a calm, predictable, comforting and safe environment.

  • Our 46 bed "Spencer Unit," an Alzheimer's/Dementia Unit, specializing in the provision of care for both higher functioning, as well as, compromised individuals under the guidance of a program director:
    • Magnetic door lock system
    • Recreation Program (7 days a week and evenings)
    • Therapeutic Courtyard
    • Structured activities on the unit
  • Utilizes an Interdisciplinary Professional Team
    • A Board Certified Psychiatrist, as part of the interdisciplinary team, provides direction as needed for medication and behavioral management
